Testosterone replacement therapy is intended for men with a well-founded diagnosis of testosterone deficiency, not for general tiredness or performance enhancement. A responsible Irish pathway should confirm symptoms, repeat low testosterone results where appropriate, assess possible causes, and discuss benefits, risks, fertility implications, and alternatives.
Before treatment is considered, doctors should review factors that may make TRT unsuitable or require extra caution. These include fertility plans, prostate concerns, high haematocrit, untreated severe sleep apnoea, recent cardiovascular events, uncontrolled heart failure, and other relevant medical history.
Treatment choices and follow-up
TRT can be delivered through different formulations, including gels and injections, each with practical trade-offs. The right choice depends on clinical suitability, patient preference, monitoring results, side effects, and access. Follow-up is necessary to review symptoms, testosterone exposure, safety bloods, and whether the treatment remains appropriate.
